Please arrive about 10 minutes early so that you have enough time to fill out a New Patient form.
Initial Examination
Your dentist will start by going over your medical and dental history, followed by a full dental exam. This includes checking your teeth, gums, bite, and jaw, doing an oral cancer screening, and taking any necessary X-rays.
Treatment Plan
After reviewing your history and completing the exam, your dentist will create a personalized treatment plan for you. They’ll also take the time to answer any questions you may have about your oral health or available treatment options.
Insurance & Payment
At Fifth Avenue Dental, we'll help you navigate our options for payment and insurance, including direct-billing your insurance company, so you can focus on achieving your dental goals.
Insurance Claims
We can bill your insurance company directly, and our front desk team is always happy to help you better understand your coverage.
Payment Methods
The full treatment cost (or the patient portion if direct-billing to your insurance) must be paid at the time of your appointment. We accept payment by cash, debit or most major credit cards.
