At Fifth Avenue Family Dental Centre we follow the current
Ontario Dental Association Suggested Fee Guide and remain consistent with fees charged in the general area.

We would appreciate prompt payment at time of service, and we offer the following payment options:
If you have dental insurance, please remember to bring all your insurance information with you and our staff will be pleased to assist you with claim forms. If your insurance plan allows, we can electronically submit your insurance claims and you will be reimbursed directly by the insurance company (sometimes as early as a few days).

Insurance Benefits

At Fifth Avenue Family Dental Centre, we believe that you deserve the best care. That's why we always present you with the best dental solution possible to treat your personal situation. Each year we provide outstanding dental care to hundreds of patients. Some have dental benefits but some don't. If you have dental benefits, congratulations! You are very fortunate. Here are some important things you should know...

Dental Benefits differ greatly from medical benefits. In 1959, most dental benefit plans had a yearly maximum cap of $1000. There has been no significant increase in the yearly maximum cap in 40 years! However, there have been significant increases in your premiums. Dental benefit plans may never pay for completion of your dental care. It is only meant to assist you. Many dental benefit plans tell their participants that they will be covered "up to 80% or 100%" but do not clearly specify the plan fee schedule allowance, annual maximums or limitations. It is more realistic to expect dental benefit plans to cover between 25% to 50% of dental services.

Almost all dental benefit plans are the result of a contract between the plan sponsor (employer or union) and the insurance company. For this reason, concerns about your dental plan should first be directed to your plan administrator. Limitations in coverage are the result of the financial commitment the plan sponsor has agreed to make and the benefit the insurance company will offer in exchange for that commitment. Treatment decisions must be made by you and your dentist. While dental benefit coverage should be taken into account, it should not be the deciding factor in your choice of treatment. Because your dental insurance is a contract between you, your employer and the insurance company we suggest that you ask your plan administrator to explain the individual design features of your plan including exclusions, limitations, patient co-payments and annual lifetime benefit maximums.
